美容医療トラブルに特化した救急外来が本格開始 東京 新宿
二重手術や脂肪吸引など美容医療のニーズが高まる一方で、一部では、合併症や後遺症などの健康被害も起きています。そうした中、東京・新宿区の病院が6月から、美容医療のトラブルに特化した救急外来を本格的に開始しました。
美容医療をめぐっては全国の消費生活センターなどに寄せられた健康被害の相談が、昨年度822件と、5年前の1.7倍に増加しています。
こうした中、東京 新宿区で24時間、救急患者を受け入れている春山記念病院は、患者を迅速に治療するため、6月から、美容医療のトラブルに特化した救急外来を本格的に開始しました。
病院はこれまでも、美容医療による合併症などの緊急手術を行ってきましたが、どんな施術を受けたのか分からず、処置に苦慮するケースも少なくなかったといいます。
このため6月には、都内の美容クリニックと覚書を交わし、クリニックで施術を受けた患者を病院が受け入れる場合は、施術内容など、治療に必要な情報を共有していくことになりました。
治療は公的な医療保険が適用されない自由診療で行われます。
美容医療の健康被害をめぐっては、厚生労働省の検討会も去年、美容クリニックと、合併症などに対応できる医療機関の連携を深める必要性を指摘しています。
覚書を交わした美容クリニックの深堀純也理事長は「これまで施術後の死亡事故や重篤な後遺症は起きていないが、今後、予期せぬ合併症が起こる可能性もあり、安心して美容医療を提供するうえで、救急病院と連携できるのは心強くありがたい」と話していました。
病院の事業責任者の櫻井裕基 医師は「一刻一秒を争う事例があるため協定を事前に結ぶ取り組みを始めた。今後はほかのクリニックとの連携も広げていきたい」と話していました。
合併症や後遺症で緊急手術も 連携の必要性は
美容クリニックで施術を受けた後、合併症や後遺症を患い、緊急手術が必要になる患者は後を絶ちません。
東京 新宿区の春山記念病院では、5月にも都内の美容クリニックであご下の脂肪吸引を受けた40代の男性患者が救急搬送されてきました。
男性は施術後に、首に血腫が出来て気道が圧迫されていました。
救急搬送にはクリニックの医師が同行し、病院側に施術内容を速やかに伝えたため、血腫の原因が首の血管からの出血であることが分かり、すぐに、血管を縛って止血する手術が行われました。
手術は1時間余りにおよびましたが無事成功し、男性は翌日には退院でき、その後の経過も順調だということです。
男性は「とても怖かったです。救急病院がなければ死んでいたかもしれず、本当に感謝の気持ちでいっぱいです」と話しています。
病院の事業責任者の櫻井裕基 医師は「去年には大阪で同じような合併症が起きて亡くなった人もいて、もう命を落とす人を出してはならないと感じ搬送を受け入れた。今回はクリニックの医師から施術内容を詳しく聞けたので迅速に処置できたが、重篤なケースでも施術内容が分からず、処置が遅れてしまうケースが出てきている。救急医療を担う病院と美容クリニックとの連携は全国で広げていくべきだ」と話しています。
国も対応強化の方針
美容医療で相次ぐ合併症や後遺症について、国も対応を強化する方針です。
厚生労働省は去年8月、美容医療の合併症や後遺症の実態調査を行い、美容医療を提供する417の医療機関と、美容医療でトラブルを経験した600人から回答を得ました。
それによりますと、まず医療機関に健康被害などのトラブルが起きた時の対応を聞いたところ、「マニュアルや研修を用意していない」と答えたところが全体の33.8％に上りました。
また、アフターフォローを行う際に、対応できない施術の修正や後遺症などが発生した場合、連携している医療機関があるか尋ねたところ、35.7％が「ない」と回答しました。
一方、トラブルを経験した患者のうち18.7％の人は、「施術を受けた後、早期に再施術や治療が必要な状況に陥った」と回答しています。
具体的な症状を複数回答で聞いたところ
▽「熱傷」が25％で最も多く
次いで
▽「重度の形態異常」が23.2％
▽「皮膚のえ死・潰瘍」が22.3％となったほか
▽「消化器障害」が3.6％
▽「骨折」や「出血多量」が2.7％などとなっています。
厚生労働省の検討会は去年、報告書をまとめ、合併症や後遺症が起きた時に、クリニックが対応できない、あるいは、対応を拒絶したり、ほかの病院の紹介も行わないといった事例が見られると指摘しました。
検討会は患者が急変した時の体制や仕組みが十分でないとして、美容クリニックなどに対応を強化するよう求めています。
具体的には、合併症などの問題が起きた時、患者が相談できる連絡先を毎年、都道府県などに報告し、行政側がそのリストを公表することも検討すべきだとしています。
また、緊急手術を担ってもらう病院と、事前に合意を交わしておくなどの連携を進め、関係学会にはその具体的な対応方法をまとめたガイドラインを策定するよう求めています。
国は今後、報告書の内容を反映した医療法の改正を目指すことにしています。
